Please look for the name of the doctor you will be seeing and choose either PDF or online form. If you download the PDF, please complete the form and bring with you on the day of your appointment to expedite your visit. If you choose the online option you will need to spend a few minutes in front of the computer. Please contact us with any questions.

Note: Urology Consultants has instituted a “no – show appointment policy.”  A fee of $30 will be charged for appointments cancelled without a full 24 hour notice.  Thank you for properly notifying us with an appropriate notice.  This allows us to give the appointment to other patients.
